Q. What is the Alpha Dental Plan?
A. Our plans are Network Dental Plans designed, owned, and administered by dentists. Our plans offer the finest professional care available at fees considerably less than you would normally pay.
Q. What is the cost of the individual Alpha Dental Plan?
A. The cost of the individual Alpha Dental Plan can be found here.
Q. Can we use any Dentist?
A. No. As an individual Alpha Dental Plan member you must choose from our dentist directory.
Q. Can I use my Dentist?
A. At this time we only offer services with the dentists in the dentist directory. If your dentist is not participating in the Alpha Dental Plan, please call to see how they can enroll.
Q. Is the Alpha Dental Plan available for group enrollment or is it just available to individuals?
A. Yes, the Alpha
Dental Plan is available for groups.

your employer to make this plan available and you will save the
$20 annual enrollment fee.
Q. Can you send me an info packet?
A. We would be happy to send you more information. Please fill out our contact form or call us and would be happy to send you additional information.
Q. Do you cover pre-existing conditions?
A. Yes, under the Alpha Dental Plan, if work has not been started.
Q. Do you offer coverage outside of Colorado?
A. Unfortunately, we do not offer dental plans outside of Colorado at this time. We are in the process of developing additional Dental Plan options for other states.
Q. Can I pay month to month or do you offer only yearly membership?
A. You must stay in the plan for one year, but can pay monthly, quarterly or annually by payroll deduction or bank draft.
Q. Where can I find information about local dentists?
A. We have a summary of the dentists available under Alpha Dental plan. You may call the dentist to get more information.
Q. Where can I get an application?
A. You may enroll online or we have a downloadable application, in Adobe Acrobat format, that you can print and mail to us. Please include a check with the individual plan application.
Q. Where do I receive treatment?
A. Our dental offices are conveniently located throughout your community. You must select a dentist from the list of plan participating dentists to be eligible for services. You may change dentists any time by contacting us.
Q. What is the maximum age of coverage for dependents?
A. Your children are eligible under the dependent coverage up to the age of 19 or full time students up to 23 years of age

A. Yes. We have a full schedule of benefits online in PDF format.
